Coordinator Community Services Aged Care

Located at our Community Services office at Mount Gambier As a Resthaven Coordinator Community Services, you’ll experience the satisfaction of supporting older people to remain living in their own homes, as you interact one-on-one with them to develop and manage care plans.

This role is offered in a Permanent Part-time capacity.

Your manager will take the time to get to know you and understand how you like to be supported as you take on this challenging role, which incorporates an on-call roster. With their guidance and support, you’ll feel confident in undertaking assessments and applying your knowledge of programs and services.

You will enjoy the mutual support and friendship of your team members and the dedicated home support workers you provide leadership to.

Salary: $86,662.40 (FTE) + Salary sacrifice + Super

Essential Criteria:

• Experience in assessment, referral, and creation/coordination of Human Services
• Excellent time management skills and ability to balance competing priorities
• An understanding of issues for older people, their families, and carers
