We are looking for an experienced Registered Nurse to join our community team based in Arncliffe. As a Community Nurse, you will build rapport with your clients in their homes while providing care in line with their personalized care plans. This includes health assessments, chronic wound care, medication administration, and offering education and support to patients and their families. In this role, you will work as a Primary Nurse, providing hands-on care to meet the needs of your patients, including: Complex wound management Palliative care support for patients and their carers Symptom management and end-of-life care Continence and catheter care Stoma care Support for the aged and those with chronic illnesses You will liaise with the patient’s health and non-health team to ensure safety at home, providing education and support to enable independence and effective management of health conditions.
